TERMS: Account balance is due in full each month unless stated otherwise. To avoid a late charge, payment in full must be made within 30 days of date of purchase. On amounts unpaid 30 days or more, a late charge will be added computed at a periodic rate of 1.5% (an annual percentage rate of 18%) and calculated from the date of purchase. In the event of default of payment, I/we agree to pay any and all costs of collection including, but not limited to, reasonable attorney fees. It is hereby certified by the undersigned that: (1) the terms of this agreement have been read and its terms and conditions are unconditionally accepted by the purchaser; and (2) all information supplied on behalf of the purchaser in the new account application is factually correct and accurate. In the event an auto­matic delivery customer runs out due to inaccurate tank size information, inability to access fill and/or discontinuance of supplemental heat without notification to the company, any related charges will be the responsibility of the customer.    NOTICE: Consumer reports (credit reports) may be obtained in connection with this appplication.  If you request, (1) you will be informed whether or not consumer reports were obtained; and (2) if reports were obtained, you will be informed of the names and addresses of the consumer reporting agencies (credit bureaus) that furnished the reports.
